Community Care Program
Our care coordinators provide in-home visits for eligible residents across our service area, helping connect older adults and families with wellness information, community resources, and appropriate referrals.
We assist individuals who are 60 years of age or older in determining their eligibility for these assistance programs and resources.
Living in a rural community should not prevent older adults from accessing the critical support they may need.
