Salary Range:$25.25 - $26.50 hourly

 

Summary of Position

​

Under the direct supervision of the Director of Senior Services, the Senior Services Professional is responsible for developing comprehensive Plans of Care and assisting in the coordination and implementation of services that promote independence, dignity, and an enhanced quality of life for older adults.

This role plays a vital part within the Public Health Department by addressing the social, emotional, and healthcare needs of aging individuals. The position directly supports the department's mission to improve community well-being and promote equitable access to essential services.

 

Education  

Required:

Bachelor’s degree in Social Work, a related social science field, or licensure as a Registered

Nurse.

Preferred:

Master’s degree in Social Work, Public Health, or a related field.

Experience

 

Required:

Minimum of one year of professional experience in any area of social work practice.

​

Preferred:

Two or more years of experience working with older adults or in community-based health or human services.

Skills and Qualities

 

Required:

Proficient in Microsoft Office Suite, including Outlook, Word, and Excel.

 

Strong verbal and written communication skills, with excellent organizational and time management abilities.

 

Proven public relations skills with a positive, approachable, and friendly demeanor.

 

Experienced in preparing and maintaining accurate records, reports, timesheets, and travel expense documentation.

 

Self-motivated and capable of working independently, often in the field with minimal supervision.

 

Holds a valid driver’s license and has access to a reliable, insured vehicle.

 

Successfully cleared background check requirements.

 

Ability to adapt and stay current with the Illinois Department on Aging’s changing policies and processes.

 

Committed to ongoing professional development, with a willingness to obtain and maintain all necessary certifications and continuing education.

 

Preferred:

 

• Knowledge of HIPAA regulations and committed to compliance in all interactions.

 

• Well-versed in local community resources and support services available to senior citizens.

 

• Experienced in navigating Medicaid and similar government assistance programs, with a strong understanding of eligibility and application processes.
